Feedback Form

This is an anonymous platform to discuss any incidents or issues that has come up during team interactions. The purpose of this is to make sure the team is a safe inclusive environment for everyone. The information will be discussed amongst the D&I committee, and if necessary, shared with board or other members involved. Since this is fully anonymous and we cannot contact you with follow-up questions, we ask that you be as descriptive as you can.
